Appointments

• Forms filled out completely
• Bring insurance card/cards to office
• Bring photo ID to office
• Bring referral if needed
• Bring copay if you have one
• For medicare only patients: you have to pya 20% of the visit when checking out.
• Bring parent or legal guardian at time of visit if patient is a minor - under 18 years of age.

Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.
