Appointments
- Forms filled out completely
- Bring insurance card/cards to office
- Bring photo ID to office
- Bring referral if needed
- Bring copay if you have one
- For medicare only patients: you have to pya 20% of the visit when checking out.
- Bring parent or legal guardian at time of visit if patient is a minor - under 18 years of age.
